PDP
What is the MetLife PDP? The MetLife PDP is a
Preferred Provider Organization. That means you
choose the dentist of your choice at the time of
treatment. You do not have to select a primary
dentist; there’s no ID card to show, or referrals
for specialty care.
How does the MetLife PDP work?With the
MetLife PDP, you receive a wide range of
benefits whether or not you and/or each eligible
dependent visit a participating PDP dentist. But,
when you visit a participating dentist, (an innetwork
dentist), you have the opportunity to
make the most of your benefit plan through
access to lower out-of-pocket expenses.
Do my dependents have to visit the same
dentist that I select?No. You and your
dependents each have the freedom to choose
any dentist, in and out-of network, at any time.
What is a participating dentist, and how do I
locate one?A general dentist or specialist who
meets MetLife’s strict credentialing standards
and accepts scheduled fees as payment in full.
There are more than 52,000 participating PDP
dentists nationwide, including 10,200 specialists.
You can get a list of participating dentists online
at www.metlife.com/dental or call (800) 942-
0854 to have a list faxed or mailed to you.
What if my dentist isn’t in the PDP? We
strongly encourage you to consider using a PDP
dentist to maximize the value of your plan. Of
course, you can visit any dentist and still receive
benefits under your plan. If your current dentist does not participate in the
PDP and you’d like to encourage him/her to
apply for membership, tell your dentist to visit
www.metdental.com , or call (877) MET-DDS9.

What is a scheduled fee?A scheduled fee
refers to the maximum charge for a service that a
participating PDP dentist may charge to eligible
PDP patients.
These fees are typically 10% to 30% below
average fees of dentists in your area, which saves
you money. Your Plan may reimburse you for all
or part of this fee. When you use a participating
PDP dentist, you are responsible only for the
difference between MetLife’s benefit payment
amount and the PDP fee for services rendered.

Can I find out how much services will cost and
what will be covered prior to treatment?
MetLife strongly recommends that you have your
dentist submit a pre-treatment estimate for
services in excess of $300. While you wait, your
dentist can get a real time pre-treatment estimate
online or over the phone in minutes detailing what
services will be covered and at what payment
level. PDP plans pay for the least expensive,
clinically appropriate course of treatment.
Therefore, licensed dental consultants review
certain services such as crowns, bridges and
periodontics for appropriateness and necessity.
Do I need an ID card?No. You do not need to
present an ID Card to prove coverage or confirm
eligibility.
